Please do not submit diagnoses, medications, insurance numbers, or other health information through this website. We will discuss your care privately by phone or in person.

We use the contact information you provide to respond to your request and, if you agree, to follow up by phone, text, or email. You can opt out of texts at any time by replying STOP. We do not sell your information.

Your health information

Once you become a client, your health information is protected under federal law, including HIPAA and, where applicable, 42 CFR Part 2 for substance use treatment records. You will receive our Notice of Privacy Practices at intake, or you can request a copy at any time by calling 602-892-0633.
